I've been going to La Siora for around ten years on and off. It's a very professional place that goes out of its way to encourage non-Japanese speaking foreigners to visit. The owner, Rie Asagiri, is a very famous domme who has made some excellent videos also about ten years ago. You can see her in the hardcore Kitagawa Pro movies (links below to some awesome branding films) though I don't think she sees clients herself anymore.

For non-Japanese speakers, the fact that they have English counseling sheets is obviously helpful. Also fairly uniquely, La Siora has its own small playrooms. Other clubs generally use love hotels, meaning more cost and potential confusion getting there for non-Japanese speakers. La Siora will ensure you get what you are looking for, be it a soft scene or something suitably brutal. Most of the dommes have their own blogs with pictures. I had some excellent sessions with Emi (a striking six footer) and Tamaki (who told me she is partial to needles and branding). Emi's blog says that she recently scored 100% in her English exam, though I have only played with her in Japanese.

Although the scene here is probably 95% male-dom, the femdom scene is still quite prolific. In Roppongi you can check out the hostess type SM bars such as Sugar Heel, Kedama, Jail, Ciel, Mistress Bar, and others. Sugar Heel's chief domme is a formidable, tall (six foot) and skilled domme called Rose Henderson, who has been professional for at least ten years also. She is fluent in English. Whilst the other clubs are welcoming, you may not get far if you don't speak Japanese. Sugar Heel's Rose and Kedama's Eve (the most skilled whip artist in Japan) also starred in many of the Kitagawa Pro movies about ten years ago.

SM-Sniper (mainly maledom) and Sniper Eve (femdom) are widely available at bookstores and list clubs.

Club Q is foreigner friendly but more a cat fight club (picture martial art blackbelts beating men to a pulp in a boxing ring) and they have an English page. Also some quite gruesome (forget SS&C) videos of guys getting torn apart (or "lynched", as they say here). The friendly owner has basic, e-mail level English.
